Abstract

The use of clinical diagnostic criteria based on a scheme-topogram allows for a comprehensive clinical assessment of both the typical and the erosive-ulcerative forms of oral lichen planus (OLP). The goal of the study was to determine the role of a 61-zone topogram in objectifying the localization frequency of lesions characteristic of the typical and erosive-ulcerative forms of OLP.
Materials and methods.
The study analyzed data from a two-center trial involving 111 patients diagnosed with the typical and erosive-ulcerative form of the oral lichen planus. Multivariate analysis of variance (MANOVA), correlation analysis, and linear regression were used as statistical tests.
Results.
In the erosive-ulcerative form of LP, the buccal mucosa was most frequently affected (79% of cases), followed by the mucosa of the mouth corners (12%), and the dorsal and/or lateral surfaces of the tongue (10%). In the typical form, the buccal mucosa was involved in 72% of cases, and the dorsal tongue surface in 28% (p<0.05). Among 61 patients (55%) — 7 men and 54 women — erosive-ulcerative lesions predominated on the buccal mucosa and retromolar area (WHO topographic codes 19, 20, 55, 56), In 14 patients (12.6%) – 3 men and 11 women – lesions involved the dorsal and/or lateral surfaces of the tongue (p<0.05).
Conclusion.
Using a 61-topographic-code color-coded scheme-topogram of the oral mucosa and vermilion border of the lips is an important criterion for objectifying the localization of papular, erosive, ulcerative, and erythematous lesions; these lesions present with variable clinical symptoms, a tendency to recur, and potential for malignant transformation. The scheme serves as a foundation for developing personalized treatment recommendations and follow-up strategies for this patient group.
