The author’s method of individual impression spoon correction in patients with diabetes mellitus

Abstract

Demographic population aging leads to a steady increase in the number of elderly and senile patients requiring orthopedic dental treatment using complete removable dentures. However, the behavioral characteristics of their organism do not always allow the orthopedic dentist to perform the full range of necessary manipulations when taking dental impressions. The aim of the study was to improve the quality of functional impressions in prosthetic treatment of patients with diabetes mellitus using an original author’s method.
Materials and methods.
A controlled study included 49 patients (aged 65—84 years) with complete edentulism and type 2 diabetes mellitus. Two groups were formed: the main group (24 patients), whose treatment was carried out using an original method for adjusting the individual impression tray; and a comparison group (25 patients), where the adjustment was performed using the traditional technique with 4—6 functional tests. The effectiveness of the proposed algorithm was assessed according to the following criteria: time (minutes) required for fitting two trays (for the maxillary and mandibular jaws); number of additional edge adjustments of the tray during a single visit; and quality of the functional impression obtained.
Results.
The use of the original method for adjusting individual impression trays made it possible to reduce the procedure duration by 1.7 times from 20 minutes to 12 minutes (p<0.001); to reduce the number of tray edge adjustments by 4 times from 2 to 0.5 (p<0.001) for the maxillary jaw. The time spent on manipulations with individual impression trays for the mandibular jaw, as well as the number of their adjustments in the main group, were similarly lower than in the comparison group by 1.5 times and 3 times, respectively (p<0.001). A high-quality impression was obtained in 92% of patients in the main group and in 84% patients of the comparison group (p=0.667).
Conclusion.
A single simplified algorithm for adjusting individual impression trays significantly reduces procedure time, decreases the number of adjustments, and lowers psycho-emotional stress in patients with polymorbid pathology.
