Состав современных зубных паст: от химических компонентов к клиническим рекомендациям (нарративный обзор)

Аннотация

Все производители зубных паст обещают всестороннюю защиту зубов и/или десны, однако часто за рекламными лозунгами скрывается сложный мир химических соединений, не всегда безопасных, эффективных и понятных потребителю. Пациенты и врачи часто имеют лишь общее представление о составе зубных паст и недостаточно понимают различия между действующими веществами. Цель данного обзора — детализировать информацию о действующих веществах зубных паст и их эффективности при различных состояниях полости рта, оценить реальные преимущества и риски каждого компонента, а также предложить критерии выбора оптимального продукта с учетом индивидуальных потребностей пациента.
Материалы и методы. Нарративный обзор проводился в соответствии с критериями качества шкалы SANRA (Scale for the Assessment of Narrative Review Articles). Поиск релевантных публикаций проводился в 7 базах данных (PubMed, EMBASE, MEDLINE, Springer, Wiley Online Library, Scopus, eLIBRARY) с использованием 28 ключевых слов и словосочетаний на английском и русском языках. Глубина поиска составила 51 год. Из 342 публикаций для первичной оценки в обзор были включены 93 исследования.
Результаты. Установлено, что биодоступность фторидов зависит от химической совместимости с абразивами. SnF2 демонстрирует преимущество над другими фторидами в отношении антимикробного и противоэрозионного действия, тогда как его клиническое превосходство по реминерализации требует дальнейших исследований. В условиях одного умеренно доказательного исследования in situ CPP-ACP генерировала наивысшие уровни биодоступного кальция среди протестированных форм, однако имеющиеся рандомизированные исследования в целом не подтверждают клинического превосходства кальций-фосфатных составов над фторидными. Наногидроксиапатит показывает противоречивые результаты и не имеет одобрения FDA в качестве антикариозного агента. Ксилит может быть частью комплексной стратегии профилактики кариеса, но не альтернативой фторидам. Индекс относительной абразивности дентина (RDA) является исключительно лабораторным показателем, который следует рассматривать как одну из переменных многофакторного процесса клинического износа, но не как единственный критерий безопасности зубной пасты. Натрия лаурилсульфат (SLS) обладает ограниченными доказанными побочными эффектами, преимущественно у пациентов с рецидивирующим афтозным стоматитом, тогда как для большинства пользователей убедительных данных о клинически значимом вреде не получено. Для увлажняющих компонентов, применяемых при ксеростомии, доказательная база остается слабой: доступные исследования имеют небольшие выборки, высокий риск систематической ошибки и не позволяют выделить преимущества какого-либо конкретного ингредиента.
Заключение. Фториды должны быть обязательным компонентом зубной пасты для большинства пациентов. Указанное на этикетке содержание фтора не всегда полностью отражает ее эффективность из-за различий в рецептуре, влияющих на биодоступность активных компонентов. Выбор оптимальной зубной пасты должен основываться на критериях индивидуальных рисков пациента, типе действующих веществ, их биодоступности и уровне доказательств эффективности.
