Ways to achieve positive dynamics in the management of patients with a chronic generalized periodontitis of mild severity

Abstract

In the daily practice of a dentist, the most common inflammatory diseases caused by periodontopathogenic microflora are periodontitis and gingivitis. Inflammatory periodontal diseases are largely secondary to the general organizational processes underlying many diseases of the internal organs. Therefore, it is obvious that it is advisable to include in the complex therapy of inflammatory periodontal diseases means that contribute to the normalization of the state of protective mechanisms of the macroorganism. To implement the task, photodynamotherapy was used according to its characteristics corresponding to 10 general principles of therapeutic and prophylactic use of physical factors (Ulashchik V.S., 1992). When electromagnetic waves of the optical range interact with a macroorganism, wave and quantum effects arise, the formation probability of which depends on the wavelength. “LED-Active 03” (Russia) was chosen for photodynamotherapy. According to modern concepts, the conduct of therapeutic measures is a stress factor for the patient. When drawing up an individual treatment plan, the threshold of an acceptable level of stress is taken into account so that the ongoing therapeutic measures do not aggravate the mental status of the patient. An important link in the treatment of inflammatory periodontal diseases is monitoring the mental status of the patient, which is necessary to create a personalized plan of therapeutic measures and, if necessary, to correct it in a timely manner, as well as to motivate the patient to maintain dental health and recovery. To assess the change in the patient’s mental status in the study, a modified version of the questionnaire by A. Custhing et al. (The impact of dental disease on social functioning, Social impact of dental disease). The aim of the study was to study the long-term results of complex treatment of chronic generalized periodontitis of mild severity using modulated diode light in the red region of the spectrum. Materials and objects of the study there were 215 patients. Inclusion criteria: diagnosis “chronic generalized periodontitis of mild severity”, minimum age 25 years, maximum age 40 years. Exclusion criteria: the presence of severe background or orthodontic pathology, poor-quality orthopedic structures, urgent conditions. All participants of the study signed a voluntary informed consent for medical manipulations/interventions. The study participants were divided into two groups, in group 1, photodynamic therapy was included in the protocol of therapeutic measures.
Conclusions.
The inclusion of photodynamotherapy in the complex therapy of chronic periodontitis contributes to the normalization of the state of the protective mechanisms of the macroorganism and facilitates the rehabilitation of patients, extending the duration of remission of the disease.
